Patient Care Coordinator Opportunity

We are seeking a detail-oriented and organized Registered Nurse to join our patient care coordination team at Community Medical Center in Missoula, MT. This is a full-time position offering a competitive salary and benefits package.

About the Position:

The successful candidate will work closely with our multidisciplinary team to coordinate patient care and achieve optimal patient outcomes. With a focus on orthopedic care, you will assess patient needs, develop individualized care plans, collaborate with healthcare teams, maintain accurate documentation, and participate in quality improvement initiatives.

Responsibilities:

  • Coordinate patient care and manage patient loads
  • Collaborate with healthcare teams to ensure seamless care delivery
  • Maintain accurate and timely documentation
  • Participate in quality improvement initiatives

Requirements:

  • Current state RN license
  • Associate's degree from an accredited nursing school
  • BLS for Healthcare Provider certification
  • One year of nursing experience preferred

Salary: $83,000 - $115,000 per year, depending on experience
