As a key member of our integrated care team, you will provide proactive, preventive care to seniors, focusing on personalized experiences and addressing the factors that impact patient well-being.

Responsibilities
  • Provide health promotion and maintenance through the diagnosis and treatment of acute illness and chronic conditions.
  • Prescribe medication, examine patients, diagnose illnesses, and provide treatment.
  • Work collaboratively with our multidisciplinary team to develop and implement patient-centered care plans.
  • Participate in quality improvement initiatives to enhance patient outcomes and experiences.
Requirements
  • A Bachelor's degree in Nursing or a related field.
  • 8 or more years of technical experience as a Nurse Practitioner or in a related field.
  • 2 or more years of project leadership experience.
  • Active, unrestricted Nurse Practitioner/PA license in the appropriate state.
Preferred Qualifications
  • Masters in Science of Nursing/PA.
