Job Title: Case Manager - Targeted Case Management

Wisconsin Community Services is seeking a skilled Case Manager to join our team in Milwaukee. As a Case Manager, you will work with adults who have serious and persistent mental illness, providing them with the support and resources they need to achieve their goals.

Job Summary:

This is a full-time position that requires a Bachelor's Degree in a behavioral science, with a preference for social work experience. The ideal candidate will have good computer skills, including Word, Excel, and the CSP Exchange, as well as good organizational skills and the ability to multitask.

Responsibilities:
  • Develop a treatment relationship with consumers and formulate treatment plans that detail treatment to be provided.
  • Provide psychosocial rehabilitation, symptom monitoring & management, and supportive counseling to consumers.
  • Monitor client's psychiatric medication regimen and compliance as directed by the physician.
  • Provide crisis intervention and supportive counseling to consumers who are in crisis.
  • Responsible for maintaining a treatment-oriented environment and assuring consistency in carrying out treatment objectives.
  • Coordinate utilization of various community resources, day programs, and outside healthcare providers.
  • Assist clients in financial planning and development of income resources.
  • Coordinate consumer access to services and all entitlement programs.
  • Provide treatment-related education and liaison with client's family.
  • Assist client in securing adequate housing and prompting and training for skills necessary to keep housing.
Requirements:
  • Bachelor's Degree in a behavioral science required, with a preference for social work experience.
  • Good computer skills, including Word, Excel, and the CSP Exchange.
  • Good organizational skills and the ability to multitask.
  • Ability to meet deadlines.
  • Some knowledge of mental illness and the ability to work with individuals diagnosed with mental illness.
Working Conditions:

This position is mobile, with time spent in the community, including transporting clients, home visits, and in the office. The ideal candidate will be able to go up and down stairs and lift and carry 20 pounds to assist case managers with routine client moves.
