JOB SUMMARY

The Hospice Registered Nurse (RN) is responsible for delivering skilled nursing assessments and developing care plans for patients and families receiving services at the Hospice House. The RN plays a crucial role in ensuring that patient needs are met as outlined in the initial comprehensive assessment and subsequent evaluations. As a vital member of the interdisciplinary team, the Hospice RN provides holistic care that encompasses spiritual, emotional, psychosocial, and physical support for patients and their families.

The individual must possess the knowledge and skills necessary to provide age-appropriate care for patients in their assigned unit. This includes understanding the principles of growth and development across the lifespan and the ability to assess and interpret data that reflects the patient's condition, ensuring that care is tailored to meet age-specific needs in accordance with departmental policies and procedures.

JOB REQUIREMENTS

Graduation from an accredited School of Nursing with an active RN license.

EXPERIENCE

Preferred experience in hospice care, nursing home, or acute care settings; a minimum of one year of nursing experience is required.

SKILLS

A strong interest in end-of-life care is essential. The candidate should demonstrate the ability to collaborate effectively within an interdisciplinary team, possess excellent communication and assessment skills, and maintain confidentiality. The RN is expected to pursue continuing education opportunities to enhance clinical competencies.

REQUIRED CERTIFICATIONS

Current Basic Life Support (BLS) certification and a valid Wyoming nursing license.

ESSENTIAL FUNCTIONS

  • Conducts initial nursing assessments, comprehensive evaluations, and ongoing assessments tailored to the needs of patients and families. The initial assessment is performed upon admission to the Hospice House, with a comprehensive assessment completed within five calendar days, addressing spiritual, emotional, physical, and psychosocial needs.
  • Collaborates with the outpatient Hospice RN to update the comprehensive assessment for patients transitioning from outpatient services, in consultation with the Interdisciplinary Team.
  • Initiates and revises the patient's Plan of Care as necessary.
  • Communicates changes in patient conditions and needs to the Primary Care Provider, nursing supervisor, and Interdisciplinary Team.
  • Ensures compliance with Medicare Conditions of Participation.
  • Provides comprehensive patient care for a designated group of patients, supported by the core hospice team and ancillary services.
  • Supervises non-licensed staff in the care of Hospice residents.
  • Coordinates with Hospice aides, overseeing their care delivery and ensuring appropriate levels of support for residents and their families.
  • Documents patient assessments upon admission, during each shift, and as needed based on changes in condition.
  • Updates the Plan of Care to include interventions that address the physical, psychosocial, emotional, and spiritual needs of residents and families, in collaboration with the attending physician, Hospice Medical Director, and the Interdisciplinary Team.
  • Assesses and documents goals and outcomes for hospice residents and their families, reporting any changes to the Interdisciplinary Team promptly.
  • All assessments will encompass areas such as pain management, dyspnea, nausea, emotional distress, and spiritual needs.
  • Participates in Interdisciplinary Team meetings bi-weekly to review and update the Plan of Care in collaboration with the core team.
  • Transcribes and implements physician orders as appropriate.
  • Administers and documents medications as prescribed, monitoring for both therapeutic and adverse effects, and providing holistic interventions as indicated.
  • Observes, documents, and reports on residents' conditions and reactions to treatments and medications, following established policies and procedures.
  • Supports residents, families, and significant others throughout the end-of-life process, addressing the stages of grief and the dying experience.

SECONDARY FUNCTIONS

  • Engages in in-service training, discharge planning, Interdisciplinary Team meetings, and staff meetings.
  • Performs other duties as assigned and deemed appropriate.
