JOB SUMMARY

The Hospice Registered Nurse (RN) is responsible for delivering expert nursing assessments and developing care plans for patients and families receiving services at the Hospice House. The RN plays a crucial role in ensuring that patient needs are met as identified in the initial and ongoing assessments. As a key member of the interdisciplinary team, the Hospice RN provides comprehensive care that encompasses spiritual, emotional, psychosocial, and physical support for patients and their families.

The individual must demonstrate the necessary knowledge and skills to provide age-appropriate care for patients in their assigned unit. This includes understanding the principles of growth and development across the lifespan and the ability to assess and interpret data relevant to each patient's status to identify their specific needs.

JOB REQUIREMENTS

Must be a graduate of an accredited School of Nursing.

EXPERIENCE

Preferred experience in hospice care, nursing home, or acute care settings; a minimum of one year of nursing experience is required.

SKILLS

Demonstrated interest in end-of-life care, ability to collaborate within an interdisciplinary team, and excellent communication, assessment, and nursing skills. Must maintain confidentiality and actively seek continuing education opportunities to enhance clinical competencies.

REQUIRED CERTIFICATIONS

Current Basic Life Support (BLS) certification and valid nursing licensure.

ESSENTIAL FUNCTIONS

  • Conducts initial and ongoing nursing assessments tailored to the needs of patients and families, ensuring a comprehensive evaluation within the first five days of admission.
  • Collaborates with outpatient Hospice RNs to update assessments when patients transition from outpatient services.
  • Initiates and revises the patient's Plan of Care as necessary.
  • Communicates changes in patient conditions to the Primary Care Provider, nursing supervisor, and interdisciplinary team.
  • Adheres to Medicare Conditions of Participation.
  • Provides holistic patient care for a designated group of patients, supported by the core hospice team and ancillary services.
  • Supervises non-licensed staff in the care of hospice residents.
  • Oversees the work of Hospice aides, ensuring appropriate care is delivered to residents and their families.
  • Documents patient assessments upon admission, during each shift, and as needed based on changes in condition.
  • Updates the Plan of Care to include interventions addressing the physical, psychosocial, emotional, and spiritual needs of residents and families.
  • Evaluates and documents the goals and outcomes for hospice residents and their families, reporting any changes to the interdisciplinary team.
  • All assessments will encompass pain management, emotional distress, and spiritual needs.
  • Participates in interdisciplinary team meetings bi-weekly to review and update the Plan of Care.
  • Transcribes and implements physician orders as appropriate.
  • Administers medications as prescribed, monitoring for therapeutic and adverse effects, and assesses the effectiveness of interventions.
  • Observes and reports any significant changes in resident conditions to supervisors and physicians as per established policies.
  • Provides support to residents and families throughout the end-of-life process, addressing stages of grief and the dying process.

SECONDARY FUNCTIONS

  • Engages in in-service training, discharge planning, interdisciplinary meetings, and staff meetings.
  • Performs other duties as assigned and appropriate.
