Job Summary

The Cody Regional Health Hospice Nurse Specialist provides skilled nursing assessment and planning for the comprehensive care of hospice patients and families served at the Hospice House. This role is an integral part of the hospice interdisciplinary team, providing holistic care including spiritual, emotional, psychosocial, and physical care to patients, families, and significant others.

Key Responsibilities
  • Perform initial nursing assessments, comprehensive assessments, and updated assessments specific to patient and family needs.
  • Initiate patient plans of care and make necessary revisions.
  • Inform primary care providers, nursing supervisors, and interdisciplinary teams of changes in patients' conditions and needs.
  • Comply with Medicare Conditions of Participation.
  • Provide total patient care for a team of patients with assistance from the core hospice team and ancillary services.
  • Direct and supervise non-licensed staff in patient care.
  • Document patient assessments, plan of care, and outcomes.
  • Attend and participate in interdisciplinary team meetings to update patient plans of care.
  • Transcribe and carry out physician orders, administer and document medications, and observe for therapeutic and adverse effects.
  • Support patients, families, and significant others through the end of life process, including the stages of grief and dying process.
Requirements
  • RN graduate of an accredited school of nursing.
  • Hospice care, nursing home, or acute care experience preferred; 1 year nursing experience required.
  • Current BLS and Wyoming licensure.
