We are seeking a skilled Social Worker to join our team at Central Carolina Hospital. As a Social Worker, you will play a vital role in providing psychosocial care to patients in our hospital setting. Your expertise will help link patients with community and agency resources, ensuring a safe and successful discharge.
Key Responsibilities:
• Provide consultation services for mental health and substance use concerns, including assessment, crisis intervention, and education for patients and families.
• Assist providers in determining appropriate patient dispositions and facilitate referrals to treatment and community resources post-discharge.
• Participate in hospital and community initiatives addressing mental health and substance abuse issues.
• Provide expertise and knowledge in mental health and substance abuse issues to colleagues during interdisciplinary rounds and team meetings.
• Fill in as a case manager/discharge planner, providing care coordination and discharge planning for primary medical patients on med/surg floors and in ICU.
Requirements:
• Bachelor's degree required; Master's degree preferred.
• LCSWA/LCSW preferred.
