As a Nurse Practitioner at WellBe, you will be responsible for providing primary care to patients in their homes, working collaboratively with our physicians, social workers, paramedics, and care team coordinators to deliver concierge-level medical care and social support. You will be accountable for caring for patients, maintaining accurate and current patient records, and scheduling initial, urgent, and follow-up appointments as required.

**Requirements:**



  • Master's Degree in Nursing
  • Valid nurse practitioner license in the state of Ohio
  • 5+ years of progressive care experience
  • BLS and ACLS certification, and an NPI# as an NP
  • A valid driver's license, car insurance, and access to a car for patient visits

**Travel Requirements:**


Travel may be required up to 100% locally, depending on patient needs.


**Work Conditions:**


The ability to lift up to 20lbs, with occasional lifting of up to 50lbs, and the ability to stand for extended periods.
