Job Description

We are seeking a compassionate and skilled Licensed Practical Nurse to join our team at Hospice of the Western Reserve. As a member of our interdisciplinary team, you will provide high-quality, patient-centered care to individuals with terminal illnesses and their families.

Key Responsibilities:

  • Assess the physical, psychosocial, and spiritual needs of patients and their families.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Provide skilled nursing services, including pain and symptom management, under the direction of the patient's physician.
  • Collaborate with the healthcare team to coordinate care and ensure seamless transitions.
  • Provide emotional support and education to patients and their families.

Requirements:

  • Licensed in the State of Ohio.
  • Experience in hospice care, home health, oncology, and/or pain and symptom management preferred.
  • Current CPR certification.
  • Valid driver's license and good driving record.
