Job Summary:

The Senior Clinical Social Work Professional will provide advanced clinical evaluations and treatment of individuals in need of medical or mental health services within a healthcare setting. As a key member of the multidisciplinary treatment team, this role assumes major clinical responsibility and provides clinical treatment without restriction to type or severity of medical illness. The position requires independent judgment in dealing with complex patient cases, operating within defined policies and procedures. As a consultant and backup to other clinicians, the Senior Clinical Social Work Professional will implement treatment plans relevant to social work issues, leveraging expertise in working with specialized disease entities or populations. Leadership and training of recent graduates, entry-level social workers, and social work students are also key responsibilities of this role. Additional duties include performing related work as required.

Responsibilities:
  • Conduct comprehensive psychosocial assessments of patients and families, securing essential data for presentation to the interdisciplinary team, and participating in decision-making and planning for treatment and aftercare.
  • Provide psychosocial counseling, with minimal supervision, to patients and families impacted by illness or injury, and facilitate compliance with medical treatment plans.
  • Coordinate and supervise the discharge planning function, including referrals to home health care services, residential health care facilities, psychiatric and medical rehabilitation hospitals, and other community supports.
  • Participate in and contribute to formal interdisciplinary rounds and conferences.
  • Prioritize in-service training to social work staff and other disciplines on selected topics within the area of expertise.
  • Lead and train social work students and entry-level social workers on assigned units and programs.
  • Complete required statistics and recording for the Social Work Department.
  • Stay abreast of developments in the field, particularly in medical specialization.
  • Represent the hospital at community meetings and act as a liaison and coordinator between community and hospital programs.
Qualifications/Requirements:
  • Minimum of 2 years of experience in professional social work in a medical, mental health, or human service setting as a member of a multidisciplinary treatment team.
  • Master's degree in Social Work (M.S.W.) from a School of Social Work accredited by the Council on Social Work Education.
  • Possession of a valid license and current registration as an LMSW or LCSW, issued by the New York State Education Department.
Estimated Salary:

$85,000 - $110,000 per annum, commensurate with experience and location.
