We are seeking a highly skilled and compassionate Senior Behavioral Health Nurse to join our team at Carle Health. As a key member of our interdisciplinary team, you will contribute to the provision and management of care to patients with complex behavioral health needs.
Key Responsibilities- Assess patient status through interview, observation, and physical assessment to identify actual problems and outcomes.
- Develop an individualized plan of care using a collaborative approach with the patient, family, and healthcare team.
- Implement appropriate interventions identified in the plan of care to achieve expected patient outcomes.
- Systematically evaluate and document patient progress toward expected outcomes/goals on an ongoing basis throughout the shift.
- Modify the plan of care based on evaluation and ensure patient safety in the hospital environment by following policy/procedures and adhering to National Patient Safety Goals.
- Current licensure in good standing as a Registered Nurse in the State of Illinois.
- Advanced reading, writing skills, and basic oral skills.
- Ability to respond appropriately to customer/co-worker and maintain confidential information.
- Basic computer knowledge, including word processing, spreadsheet, email application, and web browser.
