As a Licensed Practical Nurse (LPN) in our Senior Behavioral Health unit, you will be an integral part of a dynamic interdisciplinary team providing high-quality patient care. This role requires a strong foundation in nursing principles, excellent communication skills, and the ability to work collaboratively with patients, families, and healthcare professionals.
Responsibilities- Assess patient status through interview, observation, and physical assessment, identifying actual problems and outcomes.
- Develop and implement individualized plans of care in collaboration with the patient, family, and healthcare team.
- Utilize evidence-based interventions to achieve expected patient outcomes.
- Systematically evaluate and document patient progress toward expected outcomes/goals.
- Modify plans of care based on evaluation and patient needs.
- Complete required documentation, including patient charts and event reports.
- Demonstrate professional accountability for own practice, seeking resources and continuing self-development.
- Participate in Performance Improvement initiatives and contribute to a culture of excellence.
- Current Licensed Practical Nurse (LPN) licensure in good standing in the State of Illinois.
- Basic Life Support (BLS) certification within 30 days of hire.
- Experience specific to patient population.
- Basic computer knowledge using word processing, spreadsheet, email, and web browser.
- Multicultural sensitivity and critical thinking skills.
This role is based at Carle Foundation Hospital in Peoria, IL, and requires a commitment to providing exceptional patient care in a fast-paced, dynamic environment.
