Job Overview

The Head of Patient Care Management is tasked with overseeing and ensuring the effective execution of the patient care management program within the organization. This program encompasses several key functions including Care Coordination, Resource Utilization, Patient Management, and Transition Planning.

Key Responsibilities:
  • Establish and refine systems and procedures for care coordination, resource utilization, and transition planning at the facility level.
  • Direct the department's initiatives related to transition planning and clinical quality enhancement.
  • Assess and confirm that organizational resources are utilized efficiently and effectively.
  • Supervise the gathering, evaluation, and dissemination of financial and quality metrics pertinent to resource utilization, quality enhancement, and performance improvement.
  • Encourage interdisciplinary collaboration, promote teamwork, and advocate for service excellence.
Qualifications:
  • Education: Bachelor of Science in Nursing (BSN) or Master of Social Work (MSW); a Master's degree is preferred.
  • Minimum of 5 years of leadership experience in Case Management within an Acute Care Hospital setting.
Licenses/Certifications:
  • Valid and current license to practice as a Registered Nurse in Texas or
  • Valid Texas license as a Master's Social Worker (LMSW) is required.
  • Certification as a Case Manager (CCM), Accredited Case Manager (ACM), or Fellowship of the American Academy of Case Management (FAACM) is required.
  • LCSW is preferred.
