Job Summary

We are seeking a skilled Case Manager to join our team at Healthkeeperz, Inc. The successful candidate will provide advanced social work services for the Community Alternatives Program for Disabled Adults and Children, assisting in locating, coordinating, monitoring, and determining eligibility for social, medical, financial, and other services to meet the needs of our patients.

Core Responsibilities
  • Assess and evaluate patient needs, identifying areas for social intervention and referral.
  • Conduct initial pre-screening and assessment of patients and families, evaluating needs and resources.
  • Develop and implement social work plans of care to ensure the health, safety, and well-being of patients.
  • Collaborate with healthcare teams to ensure comprehensive patient care.
  • Provide counseling and emotional support to patients and families, strengthening their support systems.
  • Document patient progress and outcomes, ensuring accurate and timely reporting.
  • Participate in interagency conferences and serve as a liaison between patients and provider agencies.
Requirements
  • Working knowledge of basic social work principles, techniques, and practices.
  • Strong organizational, communication, listening, and assessment skills.
  • Ability to travel to other office locations as needed.
Education/Training
  • Bachelor's degree in social work or a related field from an accredited institution.
  • One year of directly related community experience in the health or medical field, preferably in case management.
  • NC Medicaid certified training program completion within 90 calendar days of employment.
Physical/Environmental Demands

This role requires sitting for extended periods, frequent stretching, talking, hearing, bending, and reaching. Occasional travel to patient residences may be required, involving stairs and potential exposure to animals or other safety risks.
