Southeastern Integrated Care LLC is seeking a highly skilled and compassionate Case Manager to join our dynamic team. As a Case Manager, you will play a vital role in providing community-based mental health and substance abuse rehabilitative services to adults with complex treatment needs.
Key Responsibilities- Develop and implement comprehensive service plans to support individuals in achieving their rehabilitative and recovery goals.
- Provide psycho-education, crisis interventions, and behavioral and substance abuse treatment interventions as needed.
- Collaborate with a multidisciplinary team to ensure seamless service delivery and coordination.
- Monitor and document the status of service users' progress and the effectiveness of strategies and interventions.
- Participate in a first responder on-call system to provide 24/7 support to service users and their natural support networks.
- Master's degree in a human service field or a related field with a minimum of 1 year of experience working with the adult MH/SA population.
- Valid NC driver's license and personal vehicle insurance coverage.
- Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
