Nurse Case Management Lead Analyst

At The Cigna Group, we are seeking a highly skilled Nurse Case Management Lead Analyst to join our team. As a key member of our healthcare team, you will be responsible for coordinating and guiding care for our customers, families, and caregivers to assist with navigating through the healthcare journey.
Key Responsibilities:
- Collaborate with customers to establish goals and interventions to meet their needs.
- Establish and document care plans in medical management systems.
- Utilize motivational interviewing, behavior change, and shared decision-making to help customers achieve optimal health and well-being.
- Empower customers with skills to enhance interaction with their providers.
- Interface with customers, family members/caregivers, providers, and internal partners to coordinate needs through telephonic, email, text, and chat interactions.
Requirements:
- Active unrestricted Registered Nurse (RN) license in state or territory of the United States.
- Minimum of two years full-time direct patient care setting as an RN required.
- Flexibility to work days or evenings based on business needs.
