Job Summary

Bridgewell is seeking a highly motivated and compassionate Direct Support Professional 2 to join our team in a residential program setting. As a key member of our team, you will provide support and assistance to individuals with disabilities, helping them to develop skills and foster independence in a home environment and in the community.

Key Responsibilities
  1. Provide support and assistance to individuals with daily living skills, including personal care, meal preparation, and household management.
  2. Develop and implement individualized support plans to promote independence and self-advocacy.
  3. Collaborate with interdisciplinary teams to ensure comprehensive care and support.
  4. Transport and accompany individuals to medical appointments, recreational activities, and other community-based events.
  5. Maintain accurate and detailed records of individual progress and support provided.
  6. Participate in ongoing training and professional development to enhance skills and knowledge.
Requirements
  1. High school diploma or equivalent required; associate's or bachelor's degree in a related field preferred.
  2. Minimum one year of experience working with individuals with disabilities; experience in a residential setting a plus.
  3. Excellent communication and interpersonal skills; ability to work effectively with diverse populations.
  4. Valid driver's license and reliable transportation.
