Job Summary:

This is a Per-Diem position for an individual who is passionate about providing quality home health care to patients in need.

As a skilled nursing professional in our Home Health department, you will be responsible for assessing, planning, and coordinating the care of patients in their homes.

Responsibilities:
  • Perform initial and ongoing assessments of patients' health status to determine their specific needs and develop individualized care plans.
  • Provide skilled nursing care including wound care, IV infusions, medication administration, and monitoring of vital signs.
  • Educate patients and their families on self-care techniques, disease management, and medication regimen.
  • Collaborate with other members of the healthcare team to provide coordinated and holistic care to patients.
  • Ensure accurate and timely documentation of patient care and progress.
  • Coordinate and communicate with healthcare providers, families, and caregivers to ensure continuity of care.
  • Adhere to all state and federal regulations, professional standards, and Maxicare Select's policies and procedures.
  • Maintain confidentiality of patient information.
Qualifications:
  • Valid and current RN license in the state of Florida.
  • Minimum of 1 year of home health or related experience.
  • Strong clinical and critical thinking skills.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and prioritize tasks efficiently.
  • Must be able to pass a background check and drug screening.
