Key Responsibilities- Administer medications and treatments as prescribed by physicians, ensuring accurate and timely delivery.
- Monitor patient health status, identifying and reporting any changes or concerns to the healthcare team.
- Collaborate with the healthcare team to develop and implement personalized care plans, ensuring patient-centered care.
- Ensure that individual service plans are followed, administering or supervising treatments as prescribed.
- Make meaningful rounds to all residents, utilizing service plans and making rounds with physicians to record visits on charts.
- Document acute episodes or significant changes in patient status, maintaining accurate and detailed records.
- Receive or place calls to physicians, following through with orders and ensuring timely communication.
- Handle the preparation and administration of prescribed medications, ordering new medications and refills as needed.
- Supervise the serving of prescribed diets and fluid intake, reporting persistent unresolved problems to the physician and/or DON or appropriate discipline.
- Timely and accurately complete admission assessments and discharge documentation, ensuring seamless transitions of care.
- Supervise residents who self-administer medication, following community and regulatory guidelines.
- Provide assistance as needed with medication reminders, bathing, grooming, dressing, escort service, and other activities of daily living.
- Perform other duties as assigned, contributing to the overall success of our healthcare team.
- Requires an RN license, with knowledge of general nursing theory and practice.
- Working knowledge of infection control, universal precautions, and OSHA standards.
- Familiarity with medical diagnoses, procedures, and accepted medical treatment patterns.
- Ability to lift at least 51 lbs on a regular basis and physically able to help residents who require physical assistance.
- Read, write, speak, and understand the English language.
