Job Description: We are seeking an experienced Hospice Chaplain to join our team as a Spiritual Care Specialist PRN. This role is responsible for providing spiritual care services to patients and families/caregivers in our hospice program. As a member of our interdisciplinary team, you will work closely with other healthcare professionals to develop individualized plans of care that address the spiritual needs of our patients.

Key Responsibilities:
  1. Conduct spiritual assessments of patients and families/caregivers to identify their spiritual needs.
  2. Develop and implement individualized plans of care that address the spiritual needs of our patients.
  3. Provide direct spiritual care to patients and families/caregivers.
  4. Serve as a liaison and support to community chaplains and spiritual counselors.
  5. Maintain records of spiritual care services utilization and related activities for quality assurance and program development.

Requirements: To be successful in this role, you must have a graduate degree from an accredited seminary or school of theology (BD, MDiv or equivalent theological degree). You should also have experience working with death and dying, individuals/family/caregiver, and be comfortable in an interfaith setting. Additionally, you must be able to network with community clergy and congregations and have knowledge of and commitment to hospice philosophy of care. The estimated salary for this position is $60,000 - $80,000 per year, depending on experience.
