Compassionate Palliative Care Specialist

The Community Healthcare of Texas is seeking a skilled and compassionate Nurse Practitioner to join our team as a Palliative Care Specialist. This role requires a strong commitment to delivering high-quality patient care and working collaboratively with our interdisciplinary team.
Key Responsibilities:- Patient Assessment and Care Planning: Conduct thorough assessments of patients to identify their unique needs and develop personalized care plans that address their physical, emotional, and spiritual well-being.
- Collaboration with IDT Members: Work closely with other healthcare professionals to ensure seamless coordination of patient care and effective communication.
- Pain and Symptom Management: Develop and implement treatment plans to manage patients' pain and symptoms, utilizing evidence-based practices and the latest research in palliative care.
- Educational and Supportive Role: Provide education and support to patients and their families, empowering them to make informed decisions about their care.
- Nursing Experience: A minimum of 3 years of experience as a Registered Nurse, with a focus on palliative care or a related field.
- Advanced Practice Nursing Certification: Hold current certification as an Advanced Practice Registered Nurse (APRN) from a recognized accrediting body.
- Knowledge and Skills: Possess comprehensive knowledge of palliative care principles, pain management, and symptom control, as well as excellent communication and interpersonal skills.
We offer a competitive salary range of $120,000 - $150,000 per year, depending on experience, as well as a comprehensive benefits package, including health insurance, retirement plan, and paid time off.
