Clinical Case Coordinator

Job SummaryThe primary purpose of this position is to coordinate the Resident Assessment Instrument (RAI) process, ensuring timeliness and completeness of Minimum Data Set (MDS), Care Area Assessments (CAAs), and Interdisciplinary Care Plans.
Key Responsibilities- Monitor and guide the completion of assessments in a timely manner
- Supervise the inputting of MDS assessments, transmission to the state, and obtaining receipt of validation
- Schedule and facilitate care plan conferences
- Organize and lead monthly Quality Management meetings
- Nursing degree from an accredited college or university
- Current, unencumbered active RN license and CPR certification
- At least 2 years of experience in Long Term Care as a Clinical Case Coordinator
- Excellent knowledge of Case-Mix, Federal Medicare PPS process, and Medicaid reimbursement guidelines
- Strong written and verbal communication skills and critical thinking abilities
- Understanding of current federal, state, and local standards, regulations, and guidelines that govern healthcare facilities
- Ability to prioritize tasks and manage multiple responsibilities
We offer a competitive salary of $75,000 - $90,000 per year, depending on experience, along with a comprehensive benefits package, including medical, dental, and vision coverage, life insurance, and disability plans.
