Job Summary

The RN Care Coordinator is a vital role at Rutland Regional Medical Center, responsible for overseeing the day-to-day management of patient care from initial contact through recurring visits. This position ensures that patient care services are timely and provided at the appropriate level of care, with the primary goal of supporting and engaging in activities that result in effective patient care and resource utilization along the care delivery continuum.

Key Responsibilities
  • Manage patient care services to ensure timely and effective delivery
  • Collaborate with healthcare teams to coordinate care and services
  • Develop and implement care plans to meet individual patient needs
  • Monitor and evaluate patient outcomes to ensure quality care
  • Communicate effectively with patients, families, and healthcare teams
Requirements
  • Graduate of an accredited school of nursing, BSN preferred
  • 2 years related clinical RN experience per specialty area
  • Nursing charge or lead experience strongly preferred
  • Experience educating a diverse population on healthcare issues highly desirable
Preferred Qualifications
  • Current Vermont RN license
  • BLS Certification through American Heart Association
  • Valid Driver's License (if conducting home visits)
  • Department specific Certifications may apply
