Job Title: Care Manager

Join Community Health Centers of the Rutland Region as a Care Manager and play a vital role in delivering high-quality patient care. As a member of our team, you will work closely with patients, families, and healthcare providers to ensure seamless transitions and optimal health outcomes.

About the Role:

The Care Manager will be responsible for developing and implementing patient-centered care plans, coordinating with healthcare teams, and providing education and support to patients and families. This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.

Key Responsibilities:
  • Develop and implement patient-centered care plans in collaboration with healthcare teams.
  • Coordinate care transitions, including hospital discharges and outpatient appointments.
  • Provide education and support to patients and families on health management and self-care.
  • Collaborate with healthcare providers to ensure timely and effective care.
  • Identify and address social determinants of health that impact patient outcomes.
  • Document patient interactions and care plans in electronic medical records.
Requirements:
  • Current Vermont RN/LPN license.
  • CPR Certification.
  • Prior experience working in a nursing position required; prior case management experience in a similar outpatient setting preferred.
  • Experience in using electronic medical records and basic keyboarding skills.
