We are seeking a skilled Licensed Practical Nurse to join our team at the VNA & Hospice of the Southwest Region in Rutland County. As a key member of our multi-disciplinary Hospice team, you will provide individualized goal-directed nursing care to patients and their families.
Responsibilities:- Utilize the nursing process to provide quality care to assigned patients, ensuring that their needs are met and their care plans are implemented effectively.
- Communicate with patients, families, and healthcare teams to ensure seamless care coordination and timely documentation of patient information.
- Implement medical plans of care, nursing interventions, and procedures as necessary to ensure safe and effective patient care.
- Perform patient and caregiver education to empower informed decision-making and promote healthy outcomes.
- Identify inter-disciplinary needs of patients and document recommendations for case managers to ensure comprehensive care.
- Coordinate patient care with physicians, providers, and team members to ensure safe and effective patient and family care.
- Prior clinical acute care nursing experience required.
- Prior Hospice experience preferred.
- Valid state driver's license required.
- Ability to pass motor vehicle record check, child abuse, criminal record check, and adult abuse check.
