Job SummaryWe are seeking a skilled and compassionate RN to join our team as a Hospice Patient Care Manager. In this role, you will be responsible for the day-to-day operations of our hospice site, ensuring the provision of high-quality care to patients and their families. You will work closely with our interdisciplinary team to develop and implement care plans, manage clinical and administrative functions, and provide direct skilled bedside nursing care.
Responsibilities- Manage the site in the absence of the Executive Director or Administrator
- Oversee clinical and administrative functions, including recordkeeping and compliance
- Respond to phone referrals and inquiries in a timely and professional manner
- Participate in on-call nursing schedule and provide direct skilled bedside nursing care
- Registered Nurse with a minimum of Associate's Degree in Nursing required
- Bachelor's Degree in Nursing preferred
- Three years' experience in direct patient care in a clinical setting required
- Minimum one year of management/supervisory experience required
- Current license to practice nursing in the state of employment
