The Patient Service Representative (PSR) plays a crucial role in supporting patient service operations at Crescent Community Health Center. This position collaborates with various departments to ensure precise documentation of patient information. Key responsibilities include the accurate collection and entry of patient demographics, efficient patient check-in processes, processing patient payments, and identifying barriers to care for effective communication with relevant personnel.


Key Responsibilities:

  • Prioritize patient care by treating each individual with respect and fairness.
  • Facilitate timely and efficient patient check-in procedures.
  • Update appointment statuses to reflect arrival and consultation times.
  • Assist patients with pre-appointment documentation, including demographic and health history forms, sliding fee applications, and privacy notices, while collecting insurance and identification copies.
  • Ensure accurate scanning and indexing of patient documents, including demographics, identification, and sliding fee paperwork.
  • Review and adhere to all patient alert notes within the Electronic Medical Record (EMR) system.
  • Collect and document all applicable fees prior to patient appointments.
  • Inform patients about their sliding fee scale discounts.
  • Conduct end-of-day procedures, including report generation and cash box reconciliation.
  • Act as an advocate for patients by providing resources to address identified barriers.
  • Assist in gathering social determinants of health data using the PRAPARE tool.
  • Serve as a backup for medical scheduling as needed.
  • Complete additional tasks and projects as assigned.
  • Commit to continuous quality improvement initiatives.
  • Stay updated with evolving technology, including software and programs.
  • Uphold and promote all organizational policies and procedures.

Qualifications: Candidates must possess a high school diploma or equivalent (GED), or have one to three months of relevant experience and/or training, or a combination of education and experience. A pre-employment drug test and background check are required.
