Key Responsibilities- Provide direct care to patients in accordance with a comprehensive care plan, ensuring their physical, emotional, and social needs are met.
- Assess and provide patient and family/caregiver education, promoting independence and self-care.
- Participate in coordination of home care services, collaborating with the interdisciplinary team to ensure seamless care delivery.
- Document patient services accurately and timely, maintaining high standards of clinical excellence.
- Participate in professional growth and development opportunities, staying up-to-date with industry best practices and regulatory requirements.
- Graduate of an accredited practical or vocational nursing program with current licensure to practice in the state of operation.
- At least 1 year of nursing experience, with 3 years preferred.
- Experience in community health, hospice, or med/surg settings is a plus.
