Job Summary

CenterWell Home Health is seeking a skilled and compassionate LPN to join our team as a Home Health Nursing Professional. As a key member of our care team, you will provide high-quality nursing care to patients in their homes, promoting independence, dignity, and comfort.

Responsibilities
  • Provide professional nursing services under the direction of a RN, adhering to state and federal regulations, and CenterWell Home Health policies and procedures.
  • Develop and implement individualized care plans, focusing on patient-centered care and promoting optimal health outcomes.
  • Conduct thorough assessments, identify patient needs, and develop effective interventions to address those needs.
  • Collaborate with the interdisciplinary care team to ensure seamless care coordination and communication.
  • Document patient progress, outcomes, and any changes in condition, maintaining accurate and timely records.
  • Participate in ongoing education and training to stay current with best practices, industry developments, and regulatory requirements.
Requirements
  • Graduate of an accredited Licensed Practical Nursing program or accredited School of Vocational Nursing.
  • Current nursing license in the practicing state.
  • Valid driver's license, auto insurance, and reliable transportation.
  • Current CPR certification.
  • Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice care.
