Job Summary

CVS Health is seeking a skilled Clinical Case Manager to join our team. As a Clinical Case Manager, you will play a critical role in driving and supporting care management and care coordination activities across the continuum of care.

Key Responsibilities
  • Assess, plan, implement, coordinate, monitor, and evaluate care management and care coordination activities.
  • Utilize advanced clinical judgment and critical thinking skills to facilitate appropriate member physical health and behavioral healthcare.
  • Coordinate psychosocial wrap-around services to promote effective utilization of available resources and optimal, cost-effective outcomes.
  • Collaborate with healthcare providers to ensure seamless care coordination and effective communication.
  • Develop and implement individualized care plans to meet the unique needs of members.
Requirements
  • Master's degree in a related field (e.g., social work, psychology, nursing).
  • 3+ years of direct clinical practice experience post-master's degree.
  • Current active, unrestricted licensure (LCSW, LMFT, or LCPC) in the state of IL.
  • Bilingual required (any language).
  • Ability to travel up to 25% of the time anywhere in the state of IL.
  • Reliable transportation and mileage reimbursement.
  • 2+ years of experience working with computers (MS Outlook, Excel, PowerPoint, and Word) and navigating multiple systems simultaneously.
Preferred Qualifications
  • Managed care/utilization review experience.
  • Case management and discharge planning experience.
  • Experience providing care to Medicaid population.
  • Bilingual preferred in Polish, Spanish, or Gujarati.
