About the Role

The Clinical Case Manager, Behavioral Health, plays a vital role in driving and supporting care management and care coordination activities across the continuum of care. This includes assessing, planning, implementing, coordinating, monitoring, and evaluating care to ensure optimal, cost-effective outcomes.

Key Responsibilities

  • Utilize advanced clinical judgment and critical thinking skills to facilitate appropriate member physical health and behavioral healthcare through assessment and care planning.
  • Coordinate with healthcare providers to ensure seamless care transitions and effective utilization of available resources.
  • Develop and implement care plans that promote effective utilization of available resources and optimal outcomes.
  • Collaborate with multidisciplinary teams to ensure comprehensive care coordination and case management.
  • Provide education and support to members and their families to promote healthy behaviors and self-management.

Requirements

  • Master's degree in a related field (e.g., social work, psychology, nursing).
  • Current, active, unrestricted licensure (LCSW, LMFT, or LCPC) in the state of Illinois.
  • 3+ years of direct clinical practice experience post-master's degree.
  • Bilingual required (any language).
  • Ability to travel up to 25% of the time within the state of Illinois.
  • Reliable transportation and a valid driver's license.
  • 2+ years of experience working with computers (MS Outlook, Excel, PowerPoint, and Word) and navigating multiple systems simultaneously.

Preferred Qualifications

  • Managed care/utilization review experience.
  • Case management and discharge planning experience.
  • Experience providing care to Medicaid populations.
  • Bilingual preferred in Polish, Spanish, or Gujarati.
