Job Summary

The Clinical Case Manager, Behavioral Health, plays a critical role in driving and supporting care management and care coordination activities across the continuum of care. This position requires advanced clinical judgment and critical thinking skills to facilitate appropriate member physical health and behavioral healthcare through assessment and care planning, direct provider coordination/collaboration, and coordination of psychosocial wrap-around services.

Key Responsibilities

  • Assess and plan care for members with complex behavioral health needs
  • Coordinate with healthcare providers to ensure seamless care transitions
  • Develop and implement care plans that promote effective utilization of available resources and optimal, cost-effective outcomes
  • Collaborate with internal stakeholders to ensure alignment with company goals and objectives
  • Provide education and support to members and their families to promote healthy behaviors and self-management

Requirements

  • Master's degree in a related field (e.g. social work, psychology, nursing)
  • 3+ years of direct clinical practice experience
  • Current Active unrestricted licensure (LCSW, LMFT or LCPC) in the state of IL required
  • Bilingual required (any language)
  • Ability to travel up to 25% of the time
  • Reliable transportation required; mileage is reimbursed per company expense reimbursement policy

Preferred Qualifications

  • Managed care/utilization review experience
  • Case management and discharge planning experience
  • Experience providing care to Medicaid population
  • Bilingual preferred in Polish, Spanish or Gujarati
