Position Overview:
In this essential clinic-based role, the NP or PA will concentrate on transitional care management for patients recently discharged from healthcare facilities. You will collaborate closely with the patient’s primary care provider (PCP) to ensure comprehensive follow-up care for a period of 30 days post-discharge.

Additionally, you may receive referrals for other high-risk patients from PCPs, necessitating more frequent follow-up appointments as required.
