The Community Health Navigator plays a vital role in supporting the social, economic, and behavioral health needs of vulnerable patients within Maui Health System and the communities we serve.
This position works collaboratively with inter-disciplinary care teams to identify goals and enable members and their families to achieve those goals through identifying local community assets, vetting their services, and making these resources accessible to patients and providers.
Key Responsibilities- Supports members and their families who are experiencing complex circumstances.

- Coordinates alternative services for patients with social and non-medical needs and where other avenues of care are more appropriate to affect optimal patient outcomes, achieve continuity and quality of care, and reduce cost.
- Patient/Family education will be a key component of success.
- Experience: Minimum one (1) year of experience in a healthcare or related field.
- Education: Bachelors degree in a business or healthcare related field OR four (4) years of experience in directly related field.
- Competent computer skills with proficiency in Microsoft Office products and ability to learn new programs to support the program.
- Ability to demonstrate flexibility and adaptability when faced with internal or external barriers, or when faced with differing points of view.
- Ability to anticipate next steps, be proactive, and collaborate with coworkers and stakeholders.
- Ability to use integrated technology platforms and virtual care coordination tools.
- Ability to use good judgment and problem-solving skills, able to effectively respond to difficult situations and resolve conflicts.
- Ability to work independently in a fast-paced environment.
- Excellent written and verbal communication skills.
- Ability to complete Motivation Interviewing Certification.
- Ability to organize and prioritize workload; effective time and project management skills in achieving program initiatives and priorities.
- Ability to successfully complete tasks with little or no supervision.
- Ability to anticipate patient and team member needs.
