The Good Samaritan Society is seeking a skilled Licensed Practical Nurse (LPN) to provide high-quality nursing care to residents in our long-term care facility. As a member of our interdisciplinary care team, you will work collaboratively with healthcare professionals to deliver exceptional patient care and support.
Key Responsibilities:- Provide professional nursing care to residents of all ages in long-term care, under the supervision of a registered nurse, advanced practice provider, or physician.
- Assess residents' physical and psychosocial needs, develop and implement individualized care plans, and document resident interactions and outcomes.
- Administer medications and treatments as ordered, and provide technical support to healthcare professionals as needed.
- Counsel residents, family members, and/or resident advocates on prevention and treatment plans, and develop self-care programs.
- Work effectively with professional and supportive personnel as a member of an interdisciplinary care team.
- Graduate from an accredited practical nursing program as a Licensed Practical Nurse.
- Associate degree in practical nursing is preferred.
- Currently holds an unencumbered Licensed Practical Nurse (LPN) license with State Nursing board and/or possess multi-state licensure privileges.
- Obtains and subsequently maintains required department-specific competencies and certifications.
