Salary Range: $30.00 - $33.30 per hour
Job Summary
We are currently seeking a compassionate and dedicated Wellness Nurse, Licensed Practical Nurse (LPN) to provide high-quality patient care to our residents. As a key member of our healthcare team, you will work closely with our interdisciplinary team to develop and implement individualized care plans that promote independence, dignity, and well-being.
Responsibilities
- Provide direct patient care to assigned residents, including administering medications, performing wound care, and monitoring vital signs.
- Develop and implement individualized care plans in collaboration with our interdisciplinary team.
- Conduct thorough assessments of residents' physical and emotional needs, identifying opportunities for improvement and developing strategies to address them.
Requirements
- Graduate of an accredited school for practical or vocational nursing.
- Valid LPN license in the state employed.
- Valid CPR certification.
- Prior skilled nursing experience preferred.
