Join AccentCare as a RN Case Manager Floater

We are seeking a skilled RN Case Manager Floater to join our team at AccentCare. As a RN Case Manager Floater, you will be responsible for providing high-quality patient care and managing cases in a fast-paced environment.

Key Responsibilities:
  • Provide patient care and manage cases in a home health setting
  • Collaborate with healthcare teams to develop and implement patient care plans
  • Conduct thorough assessments and evaluations to identify patient needs
  • Develop and implement individualized care plans to meet patient needs
  • Monitor patient progress and adjust care plans as needed
Requirements:
  • Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of agency operation
  • One (1) year experience as a RN
  • Required Certifications and Licensures: Licensed to practice as a registered nurse in the state of agency operation. Must possess and maintain valid CPR certification while employed in a clinical role. Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
