Join Our Team as a Clinical Assessment Advisor

At CenterWell Home Health, we're dedicated to providing personalized, comprehensive home care for patients managing chronic conditions or recovering from injury, illness, surgery, or hospitalization. As a Clinical Assessment Advisor, you'll play a vital role in our care teams, working alongside nurses, physical therapists, occupational therapists, speech-language pathologists, home health aides, and medical social workers to help patients rehabilitate, recover, and regain their independence.

Key Responsibilities:
  • Conduct initial and ongoing evaluations of patient needs, ensuring accurate assessments and effective care plans.
  • Collaborate with healthcare professionals to develop and implement comprehensive care plans, incorporating teaching related to health maintenance, prevention, and safety.
  • Coordinate available resources to manage care plans, ensuring stated outcomes are achieved and patient needs are met.
  • Periodically reassess patient needs and revise care plans as necessary, ensuring continuity of care.
  • Communicate effectively with team members, attending physicians, administrative staff, referral sources, and external case managers to ensure appropriateness of care and coordinate necessary changes to the plan of care.
  • Ensure patient needs are identified and qualified caregivers are assigned to all cases within your scope of practice.
Requirements:
  • Bachelor of Science degree in Nursing (BSN) preferred.
  • At least 2 years of clinical home care experience, with a minimum of 1 year in a clinical management role.
  • Current CPR certification.
  • Excellent verbal and written communication skills.
  • Excellent interpersonal skills.
  • Knowledge of state and federal home health agency regulations and compliance standards.
  • Knowledge of clinical policies and procedures and ability to implement.
  • Knowledge of clinical structure of PDGM.
  • Must read, write, and speak fluent English.
  • Must have good and regular attendance.
  • Performs other related duties as assigned.
  • A valid driver's license, auto insurance, and reliable transportation are required.
