Rehabilitation Liaison Specialist

Job Summary

The Rehabilitation Liaison Specialist receives patient referrals via phone or fax and arranges for evaluations. They review initial payor data, secure contact information, and assess patient rehabilitation potential and suitability for hospitalization. This role requires strong organizational and liaison capabilities, with well-developed written and verbal communication abilities. The ideal candidate will have two years of clinical experience in medical or one year in a physical rehab setting, with a working knowledge of Medicare guidelines for Rehabilitation.

Responsibilities

Receives patient referrals and arranges for evaluations
Reviews initial payor data and secures contact information
Assesses patient rehabilitation potential and suitability for hospitalization
Interviews patient and family to gather information and determine special needs
Adheres to admission criteria and presents potential patients for approval at admissions meetings
Coordinates pertinent data with Business and Admitting Office
Represents the facility in all contracts with other health professionals and the general public

Qualifications

Two years of clinical experience in medical or one year in a physical rehab setting
Good organizational and liaison capabilities
Well-developed written and verbal communication abilities
Demonstrated public relation abilities with a cooperative and assertive attitude
Working knowledge of Medicare guidelines for Rehabilitation
Must have an earned Bachelor's or Associate's degree from an accredited university and functional scope licensure/certification to function as a PT, OT, ST, or RN
