We are seeking a highly skilled Primary Care Specialist to join our interdisciplinary team at Landmark Health in Binghamton, NY.

The ideal candidate will have excellent clinical skills, strong communication abilities, and a passion for delivering high-quality patient care in a home-based setting.

Key Responsibilities

  • Provide comprehensive primary care services to patients in their homes, including assessments, care planning, and medical interventions.
  • Collaborate with PCPs, facility staff, and other health professionals to ensure seamless care coordination.
  • Educate patients and their caregivers on disease management, wellness, and healthcare resources.

Required Skills and Qualifications

  • Completion of accredited Nurse Practitioner or Physician Assistant program
  • Current state RN license (if NP) or NP/PA license
  • AANP, ANCC, or NCCPA certification
