About the Role
We are seeking a highly skilled and compassionate Geriatric Home Care Specialist to join our team at Wellbe Senior Medical. This role offers an exciting opportunity to work with patients in their homes, providing personalized care and support.

Job Summary
As a Geriatric Home Care Specialist, you will be responsible for delivering high-quality care to patients with chronic and acute illnesses, injuries, and conditions. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet each patient's unique needs.

Key Responsibilities
* Conduct thorough assessments of patients' medical, psychological, and social needs
* Develop and implement comprehensive care plans that address patients' physical, emotional, and spiritual well-being
* Provide direct patient care services, including administering medications, performing diagnostic tests, and conducting follow-up appointments
* Collaborate with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes
* Maintain accurate and up-to-date patient records, including progress notes, medication lists, and test results

Requirements
* Master's degree in Nurse Practitioner studies from an accredited program
* Active, unencumbered RN and NP licenses in the posted state
* Current board certification as an NP (AANC or AANP)
* BLS certification and active NPI# as an NP
* CPR certification
* DEA licensure or eligibility to apply upon hire
* Previous experience in home care, primary care, hospice, palliative care, or geriatrics preferred
