About the Role

We are seeking a highly skilled Rehabilitation Intake Coordinator to join our team at UnityPoint Health. In this role, you will be responsible for providing pre-admission intake assessments for acute inpatient rehabilitation services. You will serve as a primary facilitator for patient referrals from internal and external sources to our Inpatient Rehabilitation Unit (IRU).


As a Rehabilitation Intake Coordinator, you will interact with discharge planners, insurance carriers, physicians, patients, and family members to ensure a smooth referral process. You will also be responsible for verifying patient benefits and obtaining pre-authorizations prior to admission.


We are looking for a detail-oriented and organized individual who can work effectively in a fast-paced environment. If you have a passion for rehabilitation services and excellent communication skills, we encourage you to apply for this exciting opportunity.


Key Responsibilities
  • Conduct pre-admission referral assessments and complete required documentation in our electronic health record system.
  • Communicate with referral sources, physicians, and insurance companies to ensure a smooth referral process.
  • Verify patient benefits and obtain pre-authorizations prior to admission.
  • Provide tours of our IRU to families and other stakeholders.

Requirements
  • Graduate of an accredited school of nursing (B.S.N. degree preferred) or a graduate of a Physical or Occupational Therapy Program.
  • Licensed RN, P.T., or O.T. in the state of Iowa.
  • Minimum of 2 years of experience in rehabilitation services.
  • Computer experience required, with proficiency in Microsoft Word.
