Rehabilitation Intake Coordinator RN OT PT

UnityPoint Health is seeking a highly skilled Rehabilitation Intake Coordinator RN OT PT to join our team. In this role, you will be responsible for providing pre-admission intake assessments for acute inpatient rehabilitation services. You will serve as one of the primary facilitators for patient referrals from internal and external referral sources to IRU.
Key Responsibilities:
- Conduct pre-admission referral assessments and complete required documentation in E.H.R.
- Communicate pre-admission assessments to rehab physicians for further review and admission decision.
- Meet referred patients and families to review IRU program/services and identify goals and discharge plans prior to IRU admission.
- Provide tours of the IRU to families and other stakeholders.
Requirements:
- Graduate of an accredited school of nursing (B.S.N. degree preferred) or a graduate of a Physical or Occupational Therapy Program.
- Licensed RN, P.T., or O.T. in the state of Iowa.
- Minimum of 2 years in Rehabilitation services.
- Computer experience required. Must be able to use Word.
