Home Health Care Provider

We are seeking a dedicated Home Health Care Provider to join our team at TLC In Home Healthcare Agency. This role involves providing routine healthcare to clients in their homes, primarily in Monmouth County, NJ.
Main Responsibilities:
- Deliver high-quality individualized care to elderly, convalescent, or disabled patients
- Monitor and report changes in patient health status
- Collaborate with registered nurses to provide exceptional care
- Assist with daily living activities and personal care, including bathing, toileting, hygiene, dressing, feeding, exercises, and medication reminders, as per the client's Plan of Care
Qualifications:
- Experience in hospice, home health, or related fields
- Positive and patient demeanor
- Ability to build rapport with patients
- Capacity to handle physical workload
- Excellent written and verbal communication skills
- Must be able to understand, read, and write English
The estimated salary for this position is $35,000 - $45,000 per annum, commensurate with experience.
