">Job Description
">As a Homecare Nurse with the VNA Health Group, you will be responsible for providing patient-centered care to individuals in their homes. Your duties will include assessing patient needs, developing personalized care plans, and collaborating with other healthcare professionals to ensure seamless care delivery.
">Responsibilities

- Participate in a two-week clinical orientation program to ensure your success in this role.">
- Assess patient needs and develop effective care plans that meet their unique requirements.">
- Provide education and support to patients and their families on health management and disease prevention.">
- Collaborate with other healthcare professionals, including doctors, therapists, and social workers, to ensure comprehensive care.">
- Document patient progress and update care plans as needed.">
Qualifications
">To succeed in this role, you must possess:

- An active RN license (BSN preferred).">
- A minimum of one year of supervised RN experience, preferably in home health or med-surg.">
- Excellent communication and problem-solving skills.">
- A valid driver's license and reliable transportation.">
