About the Role

We are seeking a highly skilled Occupational Therapist to join our team at Bridge Home Health and Hospice. As an Occupational Therapist, you will play a vital role in helping our patients achieve their goals and improve their quality of life.

Responsibilities

As an Occupational Therapist, your primary responsibilities will include:

  • Evaluating patients' functional status and developing personalized treatment plans to improve their independence and mobility.
  • Conducting home assessments to identify potential hazards and barriers to independent living, and providing recommendations for modifications and adaptations.
  • Developing and implementing treatment programs to improve patients' functional abilities, including training in daily living skills, work capacity, and use of adaptive equipment.
  • Collaborating with other healthcare professionals to ensure comprehensive and coordinated care.
  • Documenting patient progress and maintaining accurate clinical records.

Qualifications

To be successful in this role, you will need:

  • A graduate degree in Occupational Therapy from an accredited program.
  • Current registration with the National Registration Examination of the American Occupational Therapy Association.
  • At least two years of clinical experience as an Occupational Therapist.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
