Job Summary

Bridge Home Health and Hospice is seeking a skilled Occupational Therapist to join our team. As an Occupational Therapist, you will play a vital role in helping patients regain their independence and improve their quality of life.

Key Responsibilities

  • Evaluate patients' functional status and develop personalized treatment plans to address their needs.
  • Conduct home assessments to identify potential hazards and barriers to independent living.
  • Develop and implement treatment programs to improve patients' functional abilities and promote independence.
  • Collaborate with healthcare professionals to ensure comprehensive care and coordinate services.
  • Communicate effectively with patients, families, and caregivers to ensure understanding and adherence to treatment plans.
  • Document patient progress and maintain accurate records.
  • Participate in quality assurance and performance improvement initiatives to ensure high-quality patient care.

Requirements

  • Master's degree in Occupational Therapy from an accredited program.
  • Current licensure as an Occupational Therapist in the state of practice.
  • Minimum of 2 years of experience in occupational therapy, preferably in home health or hospice care.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Strong problem-solving and critical thinking skills.
