As a key member of our care team, you will play a vital role in creating and maintaining plans of care for our patients and their families.
Key Responsibilities:- Develop and implement individualized care plans, ensuring seamless coordination of patient care.
- Collaborate with interdisciplinary teams to provide comprehensive care and support to patients and families.
- Provide education and support to patients, families, and caregivers on disease management, palliative care, and end-of-life care.
- Current Florida RN license and a minimum of one year of experience in an acute-care hospital environment.
- BSN or MSN preferred, but not required.
- Strong communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
