RN Triage/Field Care Specialist

Empath Health is seeking a RN - Triage/Field to provide clinical, physical, educational and emotional support to patients after-hours. The RN - Triage/Field will visit patients in their homes, long term care facilities, assisted living facilities, hospice houses or hospitals, and provide symptom relief using specialized nursing skills related to palliative and end-of-life care. The ideal candidate will have a current, unrestricted Florida Licensure as RN, a BSN or MSN degree, and a minimum of one year recent acute care experience in a Hospital or Home Health environment.
Key Responsibilities:
Perform patient assessment and manage discomfort
Provide symptom relief using specialized nursing skills
Utilize nursing interventions and teaching skills to maximize patient comfort
Attend patient deaths and provide bereavement support
Document patient services within electronic medical record
Requirements:
Current, unrestricted Florida Licensure as RN
BSN or MSN degree
Minimum one year recent acute care experience in a Hospital or Home Health environment
CHPN certification is strongly encouraged within 3 years of employment
