Job Summary

The Hospice RN Case Manager will be responsible for providing high-quality, patient-centered care to individuals and families in their homes and communities. This role requires strong communication and interpersonal skills, as well as the ability to work independently and as part of a team.

Key Responsibilities
  • Conduct comprehensive assessments of patients and families to determine their needs and develop individualized care plans.
  • Provide education and support to patients and families on disease management, symptom control, and end-of-life care.
  • Collaborate with the interdisciplinary team to coordinate care and ensure seamless transitions between settings.
  • Manage and coordinate the delivery of hospice services, including medication management, wound care, and other interventions.
  • Communicate effectively with patients, families, and healthcare providers to ensure that care is patient-centered and meets the needs of the individual.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures that promote high-quality care.
Requirements
  • Graduate of an accredited school of nursing.
  • Registered Nurse, with a License in good standing to practice in the state of Florida.
  • Minimum of one year of experience as a professional nurse within the last three years, or a baccalaureate degree in nursing and minimum of two years experience, at least one of which is in the area of public health, home care, or hospice nursing.
  • Excellent observation, verbal and written communication skills, problem-solving skills, basic math skills, and nursing skills per competency checklist.
  • Prolonged or considerable walking or standing, able to lift, position, and/or transfer patients, able to lift supplies and equipment, considerable reaching, stooping, bending, kneeling, and/or crouching, visual acuity, and hearing to perform required nursing skills.
  • Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
  • Possesses and maintains CPR certification (may not be internet-based unless the demonstration of skills was hands-on and observed by a certified trainer).
